WEBSITE DISCLAIMER & DRIVER MEDICAL TERMS & CONDITIONS
1.1 These Terms and Conditions govern your use of our website and the driver medical services provided by Syal Ltd (trading as D4 Driver Medicals).
1.2 By using our website or booking a medical, you agree to these Terms in full. If you disagree with any part, please do not use our website or services.
1.3 “We”, “us” and “our” refer to Syal Ltd, the operator of D4 Driver Medicals.
2.1 It is the responsibility of the driver to ensure the correct medical has been booked.
2.2 If we are unable to carry out the medical as planned, our liability will be limited to issuing a full refund. We will not be responsible for consequential or indirect losses.
2.3 The doctor cannot carry out a medical assessment if required items are not brought. Drivers must ensure they have:
List of medications
Glasses or contact lenses (if worn)
Proof of identity
Medical summary or records if required
No refund is due if these are missing.
2.4 If a driver fails the medical after submission to the licensing authority, D4 Driver Medicals accepts no responsibility for any consequential loss.
2.5 If a doctor determines that you are not fit to drive under DVLA standards (e.g., blood pressure > 180/100), the medical will not be passed. Requests to alter guidance are forbidden.
2.6 Drivers must provide accurate and honest information. Any losses arising from false or incomplete information are the driver’s responsibility.
2.7 If an error by D4 Driver Medicals or its doctors causes your licence to be delayed or rejected, we will either arrange a repeat medical at no cost or issue a full refund of the medical fee. Our liability is strictly limited to the cost of the medical examination.
2.8 Cancellations or re-arrangements must be notified with at least 24 hours’ notice (Monday to Friday). Requests after this time or non-attendance may incur the full fee.
2.9 Failure to attend without notice may result in a payment request and debt collection action for the missed appointment.
2.10 Independence
D4 Driver Medicals provides independent driver medical assessments. We are not a GP practice and do not offer ongoing clinical care, diagnosis or treatment.
2.11 DVLA Disclaimer
D4 Driver Medicals is an independent medical service. We are not part of, or endorsed by, the DVLA or any government agency. The term “D4” refers solely to the DVLA D4 medical form.
2.12 Refund Policy
Refunds are provided in full where a fault or administrative error occurs on the part of D4 Driver Medicals. Refunds are not offered for non-attendance, late cancellation, or failure to bring required documents.
2.13 Data and Privacy
Personal and medical information is processed in accordance with UK GDPR and is used only to manage bookings, perform medical assessments and meet legal requirements. Full details are available in our Privacy Policy.

What we collect
We collect only the minimum personal and medical information needed to arrange and perform driver medical assessments.
This includes the details you provide on our booking forms (name, contact information, and relevant medical history) together with cookies and anonymous website-analytics data.
Purpose and lawful basis
Your information is used to:
manage appointments and maintain clinical records, meet our legal and professional duties to the DVLA and GMC, and
monitor and improve our website and services. Processing is carried out under Article 6(1)(b) (contract necessity) and 6(1)(c) (legal obligation) of the UK GDPR.
How we store and share data
Records are stored securely in the UK and retained for up to seven years to meet medical-record requirements.
For booking and site-performance data, We only use trusted third-party processors, such as our website host, Wrdpress and Google Analytics.
Where any data is processed outside the UK (e.g., analytics), it is protected through UK-approved Standard Contractual Clauses or equivalent safeguards.

You may at any time request:
a copy of your personal data,
correction of inaccurate details, or
erasure where legally permissible.
